Maintain a safe, secure, and healthy environment by following standards and procedures, keeping with strict observance of the patient’s plan of care. Educates patient and family regarding patient's status and plans of care. Implement and modify appropriate interventions treatments based on patient response and status in conjunction with the plan of care. Graduate of an accredited school of nursing and at least 1 year of experience as a registered nurse or recent graduate with prior experience as an IV Certified LVN/LPN with one year professional nursing experience. Demonstrated competency in patient care standards required for safe delivery of services and infusion skill sets applicable to agency programs and service needs. An Infusion Nurse is a licensed clinician who coordinates and provides direct patient care in the home or alternate treatment site to ensure patient safety with continuity and compliance under a physician’s plan of care. All Veterans are visited by the Milwaukee VA Healthcare System's Home-Based Primary Care Team, who provide primary care case management in the foster home. Regular visits are made by our team, which includes primary care providers, nurses, social workers, dietitians, psychologists, occupational therapists and recreational therapists. The benefit package includes primary, acute and long term care services.

A Medical Foster Home can serve as an alternative to a nursing home. It may be appropriate for Veterans who require nursing home care but prefer a non-institutional setting with fewer residents. Implementation of Health Homes for Medicaid enrollees with chronic conditions was recommended by the Medicaid Redesign Team. As a result, this initiative was included in the Governor´s SFY11/12 Budget and was adopted into law effective April 1, 2011. Medical Foster Homes are private residences where the caregiver and relief caregivers provide care and supervision 24 hours a day, seven days a week. This caregiver can help the Veteran carry out activities of daily living, such as bathing and getting dressed. VA ensures that the caregiver is well trained to provide VA planned care. While living in a Medical Foster Home, Veterans receive Home Based Primary Care. Not only is the care excellent but the staff is exceptionally kind and helpful.
